These Terms of Service ("Terms") constitute a legally binding agreement between you ("User," "Client," "Patient," or "Driver") and The Accident Doctor governing your access to and use of our website, scheduling widgets, communication channels, and administrative services.
Please review these Terms carefully before proceeding. By accessing this web portal, utilizing our clinical scheduler, or communicating with us, you acknowledge that you have read, understood, and agreed to be bound by these Terms, as well as our Privacy Policy. If you do not agree to these conditions, you must immediately cease accessing our website and digital booking utilities.
1. Medical & legal disclaimers (non-emergency notice)

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, SUSPECT A LIFE-THREATENING ACUTE INJURY, CRITICAL TRAUMA, SEVERE CHEST PAINS, OR UNCONTROLLED HEMORRHAGING FOLLOWING A MOTOR VEHICLE COLLISION, PLEASE IMMEDIATELY DIAL 911 OR PROCEED TO THE NEAREST HOSPITAL EMERGENCY ROOM.
The healthcare details, local clinic coordinates, diagnostics guidelines, and automated calculators published on this website are compiled strictly for general informational, educational, and booking convenience purposes. They do NOT constitute an active emergency diagnoses, emergency room prescription, or legal-medical claim valuation. Online interactions do not establish a formal doctor-patient relationship until an on-site, physical examination has been completed by one of our certified medical practitioners.
2. Appointment scheduling, registry accuracy & regulatory declarations
When utilizing our online scheduling system to book a Commercial Driver License (CDL) Physical, DOT drug screening, or motor vehicle accident chiropractic evaluation, you represent and warrant that:
- Submission integrity: All data submitted by you about your identity, contact information, state of CDL licensure, active Arizona ZIP code, and underlying medical history is completely truthful, accurate, and current.
- Federal DOT compliance: Commercial drivers scheduling a DOT physical (under 49 CFR Part 391) certify that all declarations concerning health conditions, blood pressure medication histories, insulin dependencies, or respiratory/sleep apnea compliance are accurate.
- Legal warning: In accordance with federal FMCSA regulations, providing intentionally falsified or fraudulent health histories during a commercial driver examination to obtain a Medical Examiner's Certificate (Form MCSA-5876) constitutes a federal offense and may result in the immediate suspension or cancellation of your commercial motor vehicle operating credentials.
3. Fee structures & cancellation guidelines
By booking an on-site examination, you confirm your understanding of the following billing frameworks:
- Flat-fee DOT examinations: Fully compliant DOT physicals and certified medical cards are processed at a direct flat fee of $60. This includes on-site standard clinical testing (urinalysis for blood/protein/sugars, vision acuity, hearing tests, and health reviews). This service is not subject to insurance co-pays or deductibles.
- Auto-accident injury care (MVA): Patients seeking chiropractic alignment, spinal decompression, or therapeutic rehabilitation following an auto collision should contact our office to discuss care. Our team coordinates documentation directly with your personal injury attorney. Patients remain responsible for any clinical balances per their individual circumstances.
- Cancellation & no-show courtesy: To maintain sub-30 minute patient cycles, we request that cancellations or schedule modifications be performed at least 2 hours prior to the booked time slot. Walk-in slots are subject to local clinician availability.
4. SMS notifications, communication consent & carrier 10DLC regulations
By providing your mobile telephone number during the appointment booking process, you explicitly express consent to receive text/SMS notifications and phone inquiries from our clinics.
A. Use of SMS messaging channels
SMS reminders are restricted solely to shipping/booking reminders, clinic directions, on-call examiner delays, CDL certification renewals, or physical rehabilitation schedule updates. Message and data rates may apply depending on your cellular provider. You may opt out of future SMS alerts at any time by texting "STOP" or contacting our coordinators.
B. Strict data partitioning guarantee
We strictly respect your digital privacy. In compliance with mobile carrier regulations (10DLC), we do not share, sell, distribute, or exchange cellular phone numbers, text consent keys, or associated personal identifiers with third-party advertising companies or marketing registries.
5. Prohibited website use and activities
As a user of our online portals, you agree that you are strictly prohibited from:
- Using automated indexing systems, web crawlers, data scrapers, spiders, or commercial bots to harvest clinical pricing, doctor rosters, schedules, or patient lists.
- Submitting false information, spamming our ZIP verification tools with randomized data, or initiating fraudulent appointment blocks designed to disrupt clinic access.
- Attempting to bypass portal security configurations, inject malicious codes, script payloads, or override communication servers associated with our healthcare systems.
6. Liability limitations, indemnification & Arizona jurisdictional venues
To the maximum extent permitted under applicable law, The Accident Doctor Group, its board-certified medical examiners, attending chiropractors, clinical staff, directors, and web administrators shall not be liable for any direct, indirect, incidental, or consequential damages resulting from:
- Errors, omissions, or processing latencies present in the website text, ZIP calculators, or scheduling software.
- Your inability to access or utilize our portal when experiencing licensing deadlines or legal-claim timelines.
- Delays occurring on state highways or federal registries that impact physical examination completion.
These Terms of Service are governed, interpreted, and enforced in accordance with the laws of the State of Arizona, without giving effect to any principles of conflicts of law. Any legal dispute, arbitration, or claims proceeding originating from these Terms shall be resolved exclusively in the state or federal courts located in Maricopa County, Arizona.
7. Amendment of terms
We modify and upgrade these administrative Terms of Service dynamically to match state laws or carrier networks. Changes are immediately public upon upload to this tab. Your continued use of our website or scheduling services after terms are updated certifies your acceptance of the modern terms.
